[PATCH] semaphore: Remove __MUTEX_INITIALIZER()

Browse source 
__MUTEX_INITIALIZER() has no users, and equates to the more commonly used
DECLARE_MUTEX(), thus making it pretty much redundant.  Remove it for good.
