Fix the calculation of WLAN_PHY_MODE from dot11mode and channel width.
Take care of all possible values of dot11mode and channel width.
Remove the usage of vht_capable parameter. Also remove the
un-necessary usage of CONFIG_160MHZ_SUPPORT.
