Try to delay first heartbeat 10 second to account for factory transient voltages. If there is a reverse boost that the heartbeat catches it will suspend the usb which can triger the factory kill if the vbus voltage goes below 2V. If the voltage then comes back and doesn't decay while shutting down it will get stuck during shutdown.
