Use cyclic id allocator to avoid immediate reassignment of
same port id.
Assigning incremental port id gives more clarity over ports
assignment and makes it easy to debug problem in certain scenario.

Commit 54fdb1e448 ("PCIe: ASPM: Save/restore ASPM-L1SS
controls for suspend/resume") restores PCI_L1SS_CTL1, then
PCI_L1SS_CTL2. It should do those in the reverse order, since
the Enable bits are in PCI_L1SS_CTL1. It also restores L1SS
state before we restore the PCIe Capability potentially enabling
ASPM as a whole. Fix the issue by changing order for both save
and restore.

Enabling crash dump (kdump) includes
* prepare contents of ELF header of a core dump file, /proc/vmcore,
using crash_prepare_elf64_headers(), and
* add two device tree properties, "linux,usable-memory-range" and
"linux, elfcorehdr", which represent respectively a memory range
to be used by crash dump kernel and the header's location

Commit 1c3b63b8d8 ("usb: f_fs: Avoid use-after-free of
epfile") fixed use-after-free in ffs_func_eps_disable but
there is still a possiblilty of ffs_epfile_release going
ahead and freeing the buffer while ffs_func_eps_disable has
acquired the spin_lock and initialized local copy of epfile.
Since ffs_epfile_release doesn't have spin_lock protection
for freeing up the read buffer, fix this by putting it under
spin_lock protection as well.

If the runtime PM is disabled, call to get_sync() will fail. In this
scenario, keep device to suspend state and return the error with warn
stack. This should help balance the runtime PM count in get_sync()
failed scenario.
In addition, if the runtime PM is found to be disabled, decide to
return an error and let framework retry the transfers.

Currently, when the remove_requests function runs for ep0
it calls ep0_end_control_data as part of which endtransfer
is issued already. However, the remove_requests will proceed
further call stop_active_transfer for ep0 as part of which
endtransfer will be issued again. This is not expected as
issuing endtransfer back to back could put the controller
in an unknown state which might result in the controller
accessing invalid addresses leading to SMMU faults.
Fix this by clearing transfer started flag in end_control_data
for ep0 if ep_cmd was successful. If ep_cmd failed, then ignore
clearing the flag and exit.
